$100 one-time enrollment fee/family

$125 per child monthly membership fee

$300/month maximum fee per family

  • After completing the pre-enrollment form Dr. Lusk will follow-up with you within 24-hours to connect about next steps.

  • You’ll be officially enrolled in the monthly membership—giving you unlimited visits, same- or next-day appointments, direct access to Dr. Lusk, and whole-child care that’s always there when you need it.

  • From your very first visit, you’ll feel it: a calm, welcoming space where your child can thrive and you can exhale. With longer appointments, a doctor who truly listens, and holistic care for body, mind, and spirit—you’re no longer just getting by. You’re deeply supported.
